Background Check Consent

As a condition of volunteering or coaching with Bolts Athletic Organization, you authorize us and our designated screening provider, Bohn & Associates, to obtain a criminal background check, including a search of the National Sex Offender Registry, before you have any unsupervised contact with athletes. This check is renewed annually for as long as you remain active.
